| Producer Application For The Range Ready Program

INSTRUCTIONS: Thank you for your interest in the Boehringer Ingelheim Vetmedica, Inc. Range Ready Program. If you are interested in enrolling your qualifying animals in the Range Ready Program please carefully read the agreement terms and submit the required information requested below. By submitting the agreement you acknowledge that you will follow and meet all program guidelines and requirements. Upon receipt your Agreement will be reviewed and, if accepted, you will be notified and your account will be activated. You can then use the e-mail address (user name) and password that you provide in the form below to log onto the web site and enter the required animal information. I agree to follow all Range Ready protocols as outlined in the Health & Vaccination Protocol.
If I and/or my veterinarian have questions or concerns regarding the vaccination protocol, I agree to contact a Boehringer Ingelheim Vetmedica, Inc. Technical Services Veterinarian to discuss and reach an agreement in writing before making any change in the protocol as outlined. I also agree to complete the requested data for each animal enrolled in the Range Ready program on the forms found on the website at http://www.rangeready.com.
In return, Boehringer Ingelheim Vetmedica, Inc. agrees to warrant the health of the animal (minimum age requirement 10 months) for 6 months past the animal’s last vaccination if the animal has been enrolled in the Range Ready Program and has met the program requirements. (Please see the Range Ready Warranty for complete requirements and coverage.)
- If a qualifying animal becomes sick due to disease caused by an antigen covered in the required vaccination protocol, BIVI will reimburse the producer for treatment costs and veterinary services related to diagnosis.*
- If a qualifying animal dies due to disease caused by an antigen covered in the required vaccination protocol, BIVI will reimburse the producer for treatment costs, veterinary services related to diagnosis, and fair market value of the animal at the time of death up to a maximum of $3,500.00 per animal.**
