The following documentation is required to be on file at BIVI or recorded in the web database:
• Signed producer agreement
- Owner's name
- Mailing address
- E-mail address if available
- Contact numbers
- Ownerís premise ID number if available
• Individual animal records
- BVD Type 1 & 2 negative test result
- Unique tamper proof ID
- Birth date
- County and State of birth
• Vaccination Records
- Product trade name
- Product serial number
- Product expiration date
- Date of each vaccination
• Vaccination Exception Form (if exception to required protocol is being requested)
• Veterinary Certification Form completed and signed by producerís veterinarian
NOTE: Animals are certified Range Ready and can be advertised as such ONLY when all required information has been received or is accessible by BIVI and the producer receives a certification from BIVI.
